docs(plan): refresh Key files and Verification to match current design

AuthorityResolver now described as mirror + per-tier UPDATE loops +
first-build + pointer flip (not 'evidence classification, atomic
rebuild'). MetricsCollector gauges updated. Verification drops
evidence-classification tests, adds incremental-delta correctness and
sticky-invalidation coverage, and qualifies the no-SERVICE claim to
steady-state reads.
