* docs(plan): decompose completion-actions removal into 5 stacked work items

Convert the approved plan at docs/plans/remove-completion-actions-task-agent-as-post-approval-executor.md into the standard plan.md format for Task Dispatcher: goal summary, 5 numbered work items with linear dependencies (merge_pr tool -> end-node handoff -> runtime pipeline removal -> schema/DB migration -> docs/E2E closeout), out-of-scope, and open questions.

* docs(plan): fold Plan Review caveats into work items

Address the UX and security review feedback on PR #1592 by threading the caveats directly into the relevant work items so Task Dispatcher sees them at fan-out without cross-referencing the review thread.

Work Item 1: pr_url cross-check against end-node signal, request_human_input response verification, jq-based env parsing, structured logging, snapshot-test for the human-approval question.

Work Item 2: user-visible callouts for QA no longer merging, Coding/Research async merge latency.

Work Item 3: explicit regression descriptions for Review-Only and Plan-and-Decompose verification loss; grep-sweep check for stray merge_pr registrations.

Work Item 4: in-flight run migration UX (banner swap + one-time task.migrated notification), workflow.migrated notification format and dismissal, QA autonomy drop 4->3 behavioural-intent callout.

Adds a "Plan Review feedback addressed" section mapping each caveat to its landing point.

* docs(plan): fold architecture + correctness review caveats

Completes the round 2 feedback pass. All 4 reviewers (ux, security, architecture, correctness) on PR #1592 voted APPROVE; their caveats are now threaded directly into the relevant work items so Task Dispatcher has complete acceptance criteria at fan-out.
