feat(monitor-fsm): Sonnet brain + Haiku coder, pagination, no-tool directive

Brain adapter (driver.ts):
- Prepend 'you have NO tools' directive to systemPrompt so Sonnet doesn't
  try to call Bash/Read/etc under allowedTools: [] and exhaust turns with
  error_max_turns.
- Raise maxTurns 1 -> 2 so the final 'result' message is actually reached.
- Error path now logs msg keys/subtype/is_error/stop_reason and includes
  the configured model in the thrown message.

Coder adapter (actions/index.ts):
- delegate_to_coder accepts a model param and falls back to
  FSM_CODER_DEFAULT_MODEL or 'sonnet', passed through as --model to
  claude -p. Lets cheap fixes run on Haiku without editing workflow.
- fetch_discourse paginates via /t/{tid}/{start}.json windows so large
  backlogs don't silently truncate at the first page.
- PER_TOPIC_CAP = 40 keeps the newest N posts per topic in the action
  output; cursor still advances to highest_post_number so we don't
  reprocess. Prevents context overflow on busy threads (9518 etc.).
