* docs(plans): remove completion actions; Task Agent as post-approval executor

Plan for Space Task #75. Proposes deleting the completionActions system
wholesale and moving PR-merge execution to the Task Agent via a new
merge_pr MCP tool, with autonomy enforcement (request_human_input at
levels 1-3, auto-execute at 4+). Five-PR rollout, migration SQL, and
workflow-prompt changes all specified.

* docs(plans): address reviewer nits on completion-actions removal plan

- §1.1 step 2 and §1.3: correct the dependency reference from the
  non-existent `spaceManager` field to the real TaskAgentToolsConfig
  shape (`space: Space` + `getSpaceAutonomyLevel` resolver). No design
  change — the handler already has everything it needs.
- §4.1: add an explicit row for `task-agent-tools.ts` noting the
  approve_task tool-description string at line 1043 still refers to the
  "completion-action pipeline" and must be rewritten when Stage 3
  deletes that pipeline.
