refactor(bb_prov)!: extract default form into opt-in bb_prov_default_form component (#51)

- move prov_default_form.html from platform/espidf/bb_prov/ into new components/bb_prov_default_form/
- add bb_prov_default_form component with CMakeLists.txt, header, and .c definition
- expose bb_prov_default_form_asset (bb_http_asset_t) initialised via __attribute__((constructor))
- drop EMBED_FILES from bb_prov/CMakeLists.txt; remove _binary_prov_default_form_html_* symbols from bb_prov.c
- remove default-form fallback branch from bb_prov_start(); caller must now supply path="/" asset
- update bb_prov_start() doxygen to require caller-supplied root asset; document bare-minimum bringup via bb_prov_default_form
- add bb_prov_default_form row to README.md components table
- update CLAUDE.md provisioning ui section to reflect new design
