v1.0.4: refs CLI smoke + merge-path coverage (#28)

Tightens tsarina hits --format refs test coverage without touching
production code.

- test_cli_hits.py: +2 smoke tests for refs as an argparse choice
  (listed in --help; unknown values rejected).
- test_cli_hits_refs.py: +2 handler integration tests for the
  gene_ident_cols merge branch — patches load_observations with a
  PRAME frame carrying gene_names/gene_ids/protein_ids; asserts the
  identity columns survive into the final CSV. Companion passthrough
  test when the scan result has no ident columns.

#20 (integration tests for cached helpers) was already covered by the
v1.0.1 k-mer unification work — closing as done-by-other-work.

211 passed. Closes #14 and #20.
