v1.0.0: tsarina spanning — population-spanning CTA x HLA pMHC table (#21)

First 1.x release — public API surface is stable.

Adds the tsarina spanning subcommand and library function. Picks the
top-N most-studied CTAs (default 25, ranked by ms_cancer_peptide_count
after restriction-confidence filtering), crosses with a population-
spanning HLA panel (default IEDB-27), scores every (peptide, allele)
cell via topiary, and emits a wide pivot where each cell is the
single best-presented peptide for that (CTA, HLA) pair.

Output formats: wide (CTA rows x allele cols, peptide as cell value)
or long (one row per filled cell with peptide / length / percentile /
score / affinity).

Full CLI + API surface documented in tsarina/cli_spanning.py and
tsarina/spanning.py. Live-tested on real IEDB+CEDAR data: canonical
immunogenic peptides land in their expected cells (MAGE-A3 EVDPIGHLY
for HLA-A*01:01, MAGE-A4 SYVKVLEHV for HLA-A*24:02).

189 tests pass (+15 new).
