* feat(space): Plan & Decompose workflow generates stacked PR task chain

Update PD_TASK_DISPATCHER_PROMPT so the Task Dispatcher embeds stacked PR
instructions in each downstream task description — branch naming convention,
base branch, and dependency ordering — so coders automatically produce a
reviewable PR chain building bottom-up from dev.

The Task Dispatcher itself does not touch git; it only creates standalone tasks
via create_standalone_task with a "## Stacked PR Instructions" section that
tells each coder:
- Bottom task: branch plan/<prefix>/<slug>, base dev
- Each subsequent task: branch plan/<prefix>/<slug>, base = previous item's branch
- Dependency note: wait for the task below to have an open branch first

Evidence in report_result now records stack_prefix and stack_branches.
Update workflow description and space-chat-agent workflow guide to describe
the stacked PR chain output. Add/update unit tests covering the new prompt
structure.

* test(space): verify Plan & Decompose guidance mentions stacked PR chain

Adds a test to buildSpaceChatSystemPrompt coverage that confirms the
workflow picking guide for Plan & Decompose describes the stacked PR
chain output, complementing the built-in-workflows.test.ts coverage.

* docs: add stacked PR task chain entry to CHANGELOG 0.10.0
