review: use lazyById for cursor safety + real deadlock retry tests

- UpdateAIImageUsageCountsCommand: switch lazy(500) to lazyById(500) so
  concurrent INSERT/DELETE on ai_images cannot cause rows to be skipped
  or double-counted during iteration (LIMIT/OFFSET vs id cursor).
- DeadlockRetryConnectionTest: add two tests that exercise the retry
  path end-to-end using a subclass with an injectable runQueryCallback.
  One proves retry succeeds on transient deadlock; the other proves
  persistent deadlocks throw after exhausting maxRetries.
