fix(heimdall): expose Continue tools to local chat context and harden test races

- parse and preserve incoming OpenAI/Continue tool definitions on chat completions
- merge external tools with Heimdall and in-memory tools for native tool mode
- inject external tool definitions into prompt-based local GGUF context
- add regression tests for tool forwarding and OpenAI-compatible passthrough
