Serve HTTP transport at root path and fix Issue #35 bugs

- Change HTTP transport endpoint from /http to / (root path)
- SSE transport unchanged at /sse, OAuth unchanged at /oauth/*
- Remove /http-based OAuth .well-known aliases (now redundant)
- Fix RegisterTools pass-by-value bug: dynamic tools from unified
  config were never discovered because cfg was copied, not mutated
- Fix registerStaticTool to return bool so staticToolCount is accurate
- Fix tests to expect both execute_query and write_query as defaults
- Add isolator Docker API spec for testcontainers support
