* test/docs(modem): address timing, queue, fragment, and USB disconnect gaps

Docs changes:
- MD-0409 AC6: document single-slot pre-auth write buffer (F-011)
- MD-0409 AC7: document BLE event queue bounded at 32 entries (F-012)
- MD-0403 AC5: document indication fragment queue bounded at 64 (F-013)
- MD-0301 AC3: add known-limitation note about reactive USB disconnect
  detection latency (F-020)
- T-0101: add implementation note about missing 2-second timing
  assertion in device_tests.rs (F-010)
- T-0304 already documents hardware-only requirement (F-026)

Test changes:
- Add `MOCK_MAX_INDICATION_CHUNKS` capacity check to
  `FragmentingMockBle::indicate()` to mirror real `EspBleDriver`
- Add `t0605_indication_queue_overflow_rejected`: 65-chunk payload
  silently dropped (MD-0403 AC5 boundary+1)
- Add `t0605_indication_queue_at_boundary_accepted`: 64-chunk payload
  accepted (MD-0403 AC5 exact boundary)

Modem crate now has 97 tests (was 95).
