fix: deadlock retry correctness, usage count reporting, test isolation

- EmailTracking: skip deadlock retry when inside a transaction — MySQL
  rolls back the entire outer transaction on deadlock, so retrying the
  INSERT alone is broken. Check DB::transactionLevel() and re-throw
  immediately when > 0.
- UpdateAIImageUsageCountsCommand: track rows processed (COUNT from
  batch subquery) not rows changed (DB::update return value) — the info
  message was undercounting when rows already had the correct count.
- GitSummaryServiceTest: use file:// protocol for non-github URL test
  to avoid network dependency in unit tests.
