doc: add gen:hasRootDefinition convention, drop transition fallback

Space-defining nanopubs must declare their root via
<spaceIRI> gen:hasRootDefinition <rootNanopubURI> in the assertion.
The root nanopub self-references; updates point back at the original.
This gives stable space refs across updates with no chain walking or
ordering dependency. Nanopubs missing the predicate are not recognized
as space-defining — no transition fallback. Also renamed kpxl_terms:
to gen: throughout (same namespace, cleaner prefix).
