Start #271: MutantTranscript data model + pluggable EffectAnnotator (#301)

Stage 1 of #271: introduce MutantTranscript + TranscriptEdit data model and pluggable EffectAnnotator Protocol with LegacyEffectAnnotator wrapper. Additive only — no behaviour change. 576 tests pass. Next: sequence_diff annotator + fast-path routing.
