Fix #294, #295, #298: MT codon table, SpliceOutcomeSet serialization, exon-skip boundary reconstruction (#300)

Fix #294 (mitochondrial codon table), #295 (SpliceOutcomeSet JSON round-trip), and #298 (boundary-codon reconstruction on mid-codon exon skip). Three independent follow-ups from 2.4.0 review. New local codon_tables module (no BioPython for codon lookup, nudging #293 forward). 554 tests pass.
