feat: wire backlog handlers and tools into MCP server (#9)

- Create handlers.go with backlog tool handlers: project, item, plan, config
- handleProject: create projects with auto-derived prefixes, or list all
- handleItem: consolidated create/get/update/list item operations with multi-mode dispatch
- handlePlan: manage plans with project and item linking
- handleConfig: get/set configuration key-value pairs
- Add 4 new backlog tools to tools.go: project, item, plan, config
- Add package-level bk variable (backup disabled in tests, nil by default)
- Add comprehensive tests for all handlers: 18 new test cases covering happy paths and error cases
- All tests pass with 67% coverage, lint clean, build successful
