feat: add versioned migration system with quartermaster tables (#7)

- implement migration-based schema evolution in ApplySchema()
- add schema_migrations table to track applied migrations
- migration 1: documents table and FTS index (existing)
- migration 2: config, projects, items, and plans tables
- migration 3: add nullable project_id column to documents
- add tests for schema creation, idempotency, version tracking, and project_id
- stage .gitignore change for .code-graph/ directory
