Name fixed-offset timezones with PHP-style +HH:MM/-HH:MM

Replace all time.FixedZone("", offset) calls with a fixedZone(offset)
helper that produces properly named locations (e.g. "-04:00", "+05:30").
This lets callers distinguish parsed timezone offsets from UTC.
