docs: add clone step and symlink alternative to codex plugin install

The existing install instructions assumed the repo was already cloned,
which was not obvious from just the `cp -r` command. Add an explicit
`git clone` step to all four install locations (README.md,
plugins/codex/README.md, site/index.html, site/docs.html).

Also document symlink as an alternative to `cp -r` in
plugins/codex/README.md, so users working from a clone can let
`git pull` propagate skill updates without re-copying.
