fix(plugin): correct codex skill install path and remove dead plugin manifest

Codex does not scan `~/.codex/plugins/`, so the documented
`cp -r plugins/codex ~/.codex/plugins/revdiff` install method never worked.
Replace with skills-only copy to `~/.codex/skills/<name>/` in README.md,
plugins/codex/README.md, site/index.html, and site/docs.html.

Fix the SKILL.md script path fallback to resolve to
`$CODEX_HOME/skills/<skill>/scripts` instead of the non-existent
`$CODEX_HOME/plugins/revdiff/skills/<skill>/scripts` path, so the skills
work from any git repo.

Remove .agents/plugins/marketplace.json and plugins/codex/.codex-plugin/
manifests - both were non-conformant with codex's actual plugin format
and served no purpose given the skills-only install. Update CLAUDE.md
to reflect the new layout.
