Fix issues with create conversation flows (#108)

- Initialize conversation state when creating a conversation, so
  that get updates will not fail even if called before the first
  streaming data comes back
- Provide proper checks for the followup workflow for an existing
  conversation
- Improve documentation
- Add test coverage
