feat: support permission-mode and attachment record types

These records were previously logged as "unrecognized" on every run.
Expose them as opt-in search/dump targets instead — the attachment
records' `addedBlocks` prose (MCP server descriptions) is the most
useful searchable content hidden in them, with tool-delta names as a
bonus for auditing which tools were available in a past session.
