docs: restructure Diagnostic Orders IA and add workflow content (#8912)

Move catalog/reference-ranges/LOINC setup docs under Clinical
Configuration; move ordering guide from charting/ to labs-imaging/
as the single canonical home (matching Medications pattern); add
redirect for old URL; add new Results and Review page with
DiagnosticReport/Observation lifecycle; expand ordering guide with
end-to-end flow, ServiceRequest lifecycle, imaging vs lab, and
integration bridges; update hub page and cross-links.
