docs: Add video walkthrough and tutorial notes to Visits page on Provider; and reorganize sidebar to move questionnaires (#8862)

Add YouTube video embed, prerequisites callout, task completion guidance,
and no-code customization note to the provider Visits documentation.

* docs: move Questionnaires & Assessments under Clinical Configuration

Relocate questionnaire guides from docs/questionnaires to
administration/provider-directory/questionnaires and nest them under
Provider Directory in the sidebar.

Add redirects from /docs/questionnaires/* to the new URLs. Update
internal links across docs, blogs, and subscriptions.

Keep the same doc URLs; only the sidebar hierarchy changes so
Questionnaires & Assessments appears directly under Clinical Configuration
below Provider Directory, not nested inside it.

* docs: restore /docs/questionnaires URLs; keep section under Clinical Configuration

Move questionnaire guides back to docs/questionnaires so canonical
URLs stay /docs/questionnaires/*. Sidebar still lists Questionnaires &
Assessments under Clinical Configuration (next to Provider Directory).
Drop questionnaire path redirects and long-path redirect stubs.
