fix: harden concurrent indexer lifecycle (close cancellation, reindex dedup, reader consistency)

- Close() now cancels background goroutines via context and enforces a
  30s hard ceiling so shutdown is never blocked by a stuck embedder
- Add per-project reindexing dedup map to prevent redundant background
  goroutines, eliminating macOS flock same-process reentrance issues
- HasSentinelFiles() now uses the read-only connection for consistency
  with other read methods (GetMeta, Stats, Search)
- Add tests for goroutine dedup and Close() cancellation behavior
