fix(mcp): rename n_results to limit in semantic_search tool

The tool description already used the word 'limit' ("Increase limit beyond
10") but the actual parameter was named n_results, causing a mismatch.
Agents unfamiliar with the schema guessed 'k' (the KNN convention) instead,
triggering MCP validation errors for unexpected properties.

Renaming to 'limit' aligns the schema with the description and uses a more
intuitive, self-explanatory name.
