* docs(communications): expand async encounters with SDK walkthrough

Replace async-encounters.md with MDX that keeps the existing conceptual
sections and adds MedplumClient examples, header-only encounter note,
multi-patient mermaid diagram, and verify searches (TS/CLI/cURL).

Add named blocks to messaging-examples.ts for MedplumCodeBlock extraction.
Walkthrough blocks chain variables from prior steps for readability.
Cross-link Encounter.serviceType and Encounter.reasonCode in the billing
paragraph.

* fix(examples): close comment block for async encounters CLI/cURL snippets

The `/*` block wrapping the CLI and cURL code blocks for async encounters
was missing its closing `*/`, commenting out the rest of the file and
causing TS6133 (unused variable `messagingGroupThreadId`).
