feat(vscode-forst): add LSP status bar and Focus output command

Show port and idle/ready/error state in the status bar; tooltip lists resolved
forst binary and auto-start, with a link to open the Forst log channel. Register
forst.showOutput (Forst: Focus output) and refresh state after initialize,
failures, restart, and forst.* config changes. Document in README and ROADMAP.
