Add access log separation and configurable app syslog

  Route uvicorn access logs to a dedicated file (RotatingFileHandler)
  and optional syslog on LOCAL2, separate from app logs. Disable
  uvicorn's built-in access log to keep container stdout clean.

  Make the app-level SysLogHandler opt-in via
  DERIVA_CHATBOT_APP_USE_SYSLOG (default false). In Docker, compose
  driver: syslog already forwards stderr to the central rsyslog
  collector; enabling the Python SysLogHandler would duplicate every
  line. Non-Docker deployments set this to true.
