Add stop button, header hostname link, user tooltip, fix model ID

  Add client-initiated cancellation: AbortController in chat.js aborts
  the SSE fetch, server detects disconnect and sets an asyncio.Event,
  chat loop checks the event before each LLM/tool call and raises
  ChatCancelled. Send button toggles to red "Stop" while busy.

  Hyperlink the hostname in the header title to the catalog's HTTPS URL.
  Show user identity details (display name, full name, email, user ID)
  on hover over the user label via title tooltip. Expand /session-info
  to return full_name, email, and hostname fields.

  Rename default title from "DERIVA Chatbot" to "DERIVA Data Assistant".
  Fix model default from claude-haiku-4-5-latest (404) to claude-haiku-4-5.
  Guard _prime_ermrest_syntax against non-dict RAG results.

  Add Phase 10 design (multi-provider LLM support, RAG-only mode) to workplan.
