fix(provider): filter DocumentReference tab by current patient (#8805)

The Documents tab in the patient page was using a bare `DocumentReference`
search URL with no subject filter, causing all DocumentReference resources
in the project to appear for every patient.

Apply the same `%patient.id` substitution pattern used by the Encounters,
Medications, Devices, and Care Plans tabs so only documents belonging to
the current patient are shown.
