Add PHP ext/date test coverage and fix parsing for many date formats

Support AM/PM after time in token parser, midnight/noon keywords,
bare weekday names, weekday+next/last week+time, N weekday ago,
ordinal word offsets (e.g. "eighth day"), first/last day of month
expressions, standalone time expressions, compact YYYYMMDD dates,
compact timestamps with timezone, negative/positive large years with
time+tz, military time (MM/DD/YY HHMM), hour-24 with non-zero
minutes, T+hour-only ISO 8601, day+month without year, and weekday
prefix stripping before dates.
