Clean up test suite and extend coverage to 99.8% line / 97.9% branch

Phase 1 — Test suite cleanup:
- Replace 3 forbidden assert() calls with TEST_ASSERT (tests README rule)
- Remove 39 lines of duplicated serialization helpers in
  test_intrusive_future_notify_destroy.c (use cy.c originals)
- Rename gather_bytes to flatten_fragments for consistency
- Suppress pre-existing cppcheck false positives

Phase 2 — Coverage improvements (cy.c):
- Exhaustive OOM sweep tests for cy_advertise, cy_subscribe, pattern
  subscriptions, and gossip-triggered topic creation
- Ordered subscription edge cases: large tag gaps, OOM during
  reordering slot allocation, duplicate interned messages
- Disposed subscriber message delivery skip
- Reader extent growth on multiple subscribers
- Gossip pinning of topics with existing pub_writer
- NACK from unknown remote, response ACK unicast failure
- Name utility edge cases (join overflow, non-homeful resolve)
- Dedup GC after session lifetime, dedup allocation failure
- Priority set with out-of-range value, advertise_client OOM,
  home/namespace set OOM, publish OOM on subject writer

4 remaining uncovered lines are provably dead defensive code.
