fix: detect HTTPS scheme for LocalForward proxy targets

Port 443 and https:// prefixed remote hosts are automatically
proxied using TLS over the SSH tunnel (InsecureSkipVerify to
handle self-signed certs on internal services). Scheme is
propagated from ssh_config parsing through the connect request
down to the reverse proxy transport.
