Replace CY_SUBJECT_ID_MODULUS_17bit with CY_SUBJECT_ID_MODULUS_16bit (57203)

Cyphal/CAN v1.1 finalized with 16-bit subject-ID fields instead of 17-bit.
Update the modulus constant, validation bounds, documentation, and all tests.
Regenerate colliding topic hashes for migration/lifecycle tests.
