v3: harden identifiers, proxy support, and docs follow-ups (#449)

## Summary
- fix generated identifier sanitization for dotted content type ids
- add proxy and rawProxy support for remote model loading
- add regression coverage for external-only resource links
- clarify docs for response aliases, link resolution, and schema-only
remote generation
