[REF] Organize benchmark files into per-problem subdirectories

Move from flat benchmark/{linop}_{problem}_{device}.json to
benchmark/{problem}/{linop}_{device}.json. Shorter filenames,
grouped by problem. Move save_environment_info to benchmark_utils.
