refactor: clean up duplicated code, dead code, and inconsistencies

Backend:
- Remove unused legacy session aliases (SetWebSocket, DetachWebSocket,
  GetWebSocket, WebSocketDetached) that were superseded by multi-conn API
- Pass grace period from config to NewSession instead of hardcoded constant
- Fix numbered step comments in connect.go
- Fix README API endpoint /ws/terminal → /ws to match actual route
- Fix docs/backend.md package tree nesting for pkg/token

Frontend:
- Extract shared FormFields, validateForm, defaultFields, buildConnectRequest,
  matchProfile into new utils/form.ts (was duplicated across 3 components)
- Simplify AppState type to only include used states (idle, connecting)
- Separate view routing state (sessions/logs) from connection state
- Merge two keydown useEffect handlers in Terminal.tsx into one
- Remove obsolete Feature ③④⑦⑧ number comments
- Update frontend/README.md directory structure
