refactor: move rag/__init__.py implementation into rag/tools.py

  Extract register() and get_rag_store() from the rag package __init__ into
  rag/tools.py, following the same pattern as the tools/ subpackage. __init__.py
  becomes a thin re-export. Update patch targets in test_rag_tools.py accordingly.

  Consolidate coverage configuration into pyproject.toml and delete .coveragerc
  so that Coveralls picks up the same exclude_lines and omit rules as local runs,
  closing the 92% local / 88% Coveralls gap.
