Fix order-dependent external \$ref deduplication (issue #116)

When RecursiveDirectoryProvider discovers a referencing schema before the
referenced file, the generator now eagerly processes the referenced file
with its canonical class name/path and registers it in a new
processedFileSchemas registry on SchemaProcessor. Subsequent $ref
resolutions and top-level provider iterations both consult this registry,
so every reference site receives the same PHP class regardless of
discovery order.

Also adds test coverage for composed-required-promotion edge cases
(anyOf root-property required in all branches, if/else-only not promoted).
