chore(db): consolidate all migrations into single initial schema

Delete 14 incremental migrations (002-014) and merge everything into
001_initial-schema.cjs. The consolidated migration includes all tables,
columns, indexes, and extensions from the full migration history.

Existing databases with the pgmigrations table will not re-run this
migration. Fresh deployments get the complete schema in one step.
