docs(modem): close 11 validation gaps in modem spec trifecta (#362)

Add missing acceptance criteria to requirements:
- MD-0300 AC4: peer table empty after RESET
- MD-0301 AC3: ESP-NOW frames discarded during USB disconnect

Add 11 new test cases (T-0625 – T-0635) to modem-validation.md:
- T-0625: tx_fail_count increment on send failure (MD-0202, MD-0303)
- T-0626: peer table cleared on channel change (MD-0206)
- T-0627: ESP-NOW resumes after channel scan (MD-0207)
- T-0628: peer table cleared on RESET (MD-0300)
- T-0629: ESP-NOW frames discarded during USB disconnect (MD-0301)
- T-0630: BLE relay boundary preservation under rapid writes (MD-0401)
- T-0631: concurrent fragmented indications not interleaved (MD-0403)
- T-0632: Just Works BLE fallback (MD-0404)
- T-0633: BLE advertising off after RESET (MD-0407, MD-0412)
- T-0634: Write Long reassembly (MD-0409)
- T-0635: BLE_ENABLE/BLE_DISABLE idempotency (MD-0413)
