docs: reorganize sidebar for clarity and discoverability (#8717)

* docs: reorganize sidebar for clarity and discoverability

Restructure the documentation sidebar to use clearer, more intuitive
section names and groupings:

- Rename "Model" to "Clinical Workflows"
- Rename "Manage" to "Security & Identity"
- Rename "Advanced" to "Deploy & Operate"
- Move FHIR Basics and Medplum App into Get Started
- Move Migration to Deploy & Operate (alongside Self-Hosting, Compliance)
- Move Analytics into Build
- Promote Contributing to its own top-level section
- Add subheaders to Integrate (Connectivity, Clinical Partners, etc.)
- Add missing sidebar entries for scheduling/timezones and candid-health
- Demote single-page Rate Limits from category to doc link
- Update home.mdx prose to reference new section name

* docs: move migration to get started, promote deploy & operate, add self-hosting link

- Move "Migrating to Medplum" back to Get Started for replatforming customers
- Move "Deploy & Operate" section up (after Build) since hosting decisions happen early
- Move Rate Limits to Reference (it's API reference material)
- Add self-hosting link to home.mdx landing page

credentials) and reads naturally alongside other clinical workflow
sections.

Use administration/provider-directory for autogenerated items so
Docusaurus does not nest a second collapsible from the parent folder.
