* Use MedplumCodeBlock for message attachments doc

- Add createBinaryForMessage, messageWithAttachment, messageWithTextAndAttachment blocks to messaging-examples.ts
- Wire sending-messages-and-attachments.mdx to extract code from examples so it compiles
- Use object form createBinary() and add file/threadHeader scope for type-checking

- Upload a file: prose only, point to createAttachment and Binary data
- Send message with attachment: use messageAttachment block from binary-data.ts
- Add :::note for raw Binary (getReferenceString) vs createAttachment
- messaging-examples: remove createBinaryForMessage and messageWithAttachment
- messageWithTextAndAttachment: use createAttachment instead of createBinary
