[REF] Hooks backend: compute per-module, translate to group convention

The hooks backend now computes covariances per module (keyed by module
usage key), then translates to parameter group keys via
_translate_module_to_group_covariances. This avoids registering
multiple hooks on the same module when separate_weight_and_bias splits
W and b into separate groups.

Extract _detect_per_module_usages from compute_parameter_mapping.
_module_mapping stores the raw per-module list (one entry per module,
always joint W+b). _mapping stores the parameter group structure.

Also move conflicting biases check from IO collector to
compute_parameter_mapping, remove _verify_weight_bias_compatibility.
