feat: add startup hooks and reorganize config structure (#109)

- Add StartupHook type and LifecycleConfig to hold startup/shutdown hooks
- Startup hooks run sequentially before server starts; failure prevents startup
- Reorganize Config into logical groups: TLSConfig, LifecycleConfig, ExtensionsConfig
- Update all examples and tests for new config structure
