[REF] Introduce ParameterUsage dataclass for KFAC layer-parameter mapping

Unify KFAC's layer-to-parameter mapping into a single ParameterUsage
dataclass bundling operation type, parameter roles, and hyperparameters.
Rename mapping keys from "weight"/"bias" to "W"/"b" (matching
AffineLayerInfo notation), and change _mapping from dict to list.
