[REF] Migrate internal params from list[Parameter] to dict[str, Parameter]

Unify parameter representation on dict keyed by fully-qualified name,
aligning with torch.func's native dict support. Public API unchanged
(list[Parameter] still accepted). Tighten model_func type to Module.
KFAC _mapping values change from int positions to str names.
