* docs: update README with web_api, correct commands and thresholds

- Add web_api app to data flow, apps table, and project structure
- Add HTTP API section with route table and quick-start URLs
- Fix `mix test --app <name>` → `mix test apps/<name>/test`
- Fix coverage thresholds (kanban_domain 70%, usecase 50%, web_api 80%)
- Add Dialyzer to CI pipeline description
- Expand test structure section with web_api tests
- Update Architecture Evolution to include web_api addition
- Add GEMINI.md reference in Contributing section

- Add web_api to apps list (Plug + Bandit REST adapter with OpenAPI)
- Fix persistence description: it is the active Agent layer, not a placeholder
- Expand data flow to show HTTP layer through to domain
- Add web_api: 80% to coverage thresholds
